This website, raymareyes.com, is published by Rayma Y. Reyes, MSN, APRN, FNP-C, a board-certified nurse practitioner licensed in South Carolina and the founder of Momento Medical, located at 138 Milestone Way, Suite B, Greenville, SC 29615. By using this site you agree to these terms. If you do not agree, please do not use the site.

Information, not medical advice

Everything on this site, including descriptions of credentials, areas of care, and the way Rayma practices, is general information about her professional background. It is not medical advice, diagnosis, or treatment, and it is not a substitute for a visit with a licensed clinician who knows your history.

Reading this site, following a link from it, or contacting us through it does not make you a patient and does not create a provider-patient relationship. That relationship begins only when you enroll with Momento Medical and complete its intake process.

If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not rely on this website or on email for urgent needs.

Momento Medical membership

This site describes Momento Medical's direct primary care model in general terms. The actual membership agreement, fees, what is and is not included, cancellation, and the practice's own website terms are set out in the Momento Medical Terms of Service at momentomedical.com/terms.html. If anything on this site seems to differ from those terms, the Momento Medical terms control for anything related to membership.

Ownership of content

The text, photographs, logo, and design of this site belong to Rayma Y. Reyes or are used with permission. You may view the site and share links to it. You may not copy, republish, or use the content or images for commercial purposes without written permission. Trademarks and names of third parties, including certification bodies and professional associations, belong to their owners and are mentioned only to describe credentials accurately.

Acceptable use

You agree to use this site only for lawful purposes and in a way that does not interfere with it. In particular, you will not:

• attempt to gain unauthorized access to the site, its host, or any related system;
• use automated tools to scrape the site in a way that burdens it or to harvest contact details;
• introduce malicious code or probe the site for vulnerabilities without permission;

Nothing in these terms limits liability that cannot be limited under South Carolina law, and nothing here changes the duties owed to established patients under the Momento Medical membership agreement.

Governing law

These terms are governed by the laws of the State of South Carolina, without regard to its conflict of law rules. Any dispute about this site will be brought in the state or federal courts located in Greenville County, South Carolina, and you agree to that jurisdiction.

We may revise these terms when the site or the law changes. The date at the top shows the latest revision. Continued use of the site after a change means you accept the updated terms.
